The best and most accurate way to measure a dog's neck is to snugly wrap a dressmaker's measuring tape around the center of the dog’s neck, then add two fingers underneath.
The next best way is to simply measure the collar the dog is wearing from buckle to most used hole.

What is the approximate age, health and behavior of the dog?
A puppy, like a small child, will surely grow out of their first collar quickly, but needs something comfortable so that they can get accustomed to wearing clothes and walking on a lead. A puppy’s owner however , will want something special to commemorate puppyhood in pictures and maybe even bronzing in the future when puppy has out grown their first collar.
An older dog or a dog with inherent back problems may need to be walked with a halter. We have special measurements for our harnesses and recommendations from Dr. Colleen Smith, who is not only a veterinarian, but also is a certified veterinary acupuncturist, on how to evaluate whether or not you should choose a harness instead of a collar.
Dogs that pull on the lead will probably pull even harder in a regular harness and may need a sensation harness to aid in training and prevent injury to the owner , but there is never a substitute for Good Behavior Training.
